Implementation of Medication Reconciliation on Misys-CPR at the Queens Health Network

December 2005: Negley, Ott & Associates, Inc.'s consulting team supports implementation of on-line Medication Reconciliation using the Misys-CPR system.

With the support of NOA’s Cynthia Herold, MT (ASCP) and Dar Sabatino, R.N., the Queens Hospital Center, a member of the Queens Health Network and the New York City Health and Hospitals Corporation went live with automated, on-line documentation of Medication Reconciliation.

The Joint Commission on Accreditation of Healthcare Organization’s (JCAHO) has mandated that by January 1, 2006 all hospitals must have a policy in place that accurately and completely reconciles medications across the continuum of care.  This includes having a record of all the patient’s current medications including over the counter drugs.  This information must be communicated between services and providers as well as to other facilities when the patient is being transferred.

The project involved leveraging the Misys CPR™/ Misys CPOE™ system via customized configuration to pull together information regarding the current active medications being given in the hospital as well as all active prescriptions and over the counter medications the patient had at home.  By documenting this information in the electronic health record, the information can be easily viewed by all providers.

This go-live event is another significant accomplishment for the Queens Hospital Center and the Queens Health Network.

Congratulations to Queens Health Network and NOA's project team for a successful deployment!  This deployment is another example of quality and excellence towards community health by using the Misys CPR™ / Misys CPOE™ integrated electronic health record.
