December 2004: Negley, Ott & Associates, Inc.'s
Cynthia Herold, Stacie Barrow and Dar Sabatino implement
automated Discharge/Transfer Summary Documentation at the
Queens Health Network.
With the support of NOA’s Cynthia Herold,
MT (ASCP), Stacie Barrow and Dar Sabatino, R.N., the
Queens Hospital Center, a member of the
Queens Health Network and the New York Health and Hospitals Corporation
went live December 7, 2004 with automated, on-line Physician
Discharge/Transfer Summary documentation for the Inpatient and Emergency
Department.
Elmhurst Hospital Center started this process February, 2004 with a
pilot project with the Medicine Department. After the pilot, the
Discharge Summary was phased in house wide at Elmhurst and then in
December, implemented house wide at Queens Hospital Center.
The Discharge/Transfer Summary includes physical examination findings as
well as the most recent laboratory results, patient allergies, discharge
medications, diet, patient instructions, follow-up appointments and the
diagnosis for the visit. If the patient is being transferred to another
facility, the documentation also includes specific information regarding
the patient’s condition, consent for transfer and receiving facility
information.
During the month of December over 900 patients had Discharge/Transfer
documentation performed in the automated on-line medical record.
This go-live event is another significant accomplishment for
Queens Health Network.

another example of how NYCHHC continues to demonstrate quality and
excellence towards community health by using the Misys CPR™ / Misys
CPOE™ integrated electronic health record.
