March 2003:  Negley, Ott & Associates, Inc.'s Cynthia Herold and other NOA consulting staff implement Adult Medicine Clinic Progress Notes at the Queens Health Network.


Queen Hospital Center, a member of the Queens Health Network, went live March 17, 2003 with on-line progress note documentation for the Adult Medicine Clinic. Until that time all notes for the adult primary care outpatient clinics were done in the paper record.

The Adult Medicine Clinic progress note includes comprehensive documentation about the patient's past medical and surgical history, family health history, allergies, review of systems, physical
examination findings as well as an assessment and plan of care for the patient.  In addition to the documentation done by the providers, the nursing staff began documenting intake, patient education, initial nutritional and psychosocial screening, immunization administration
and nursing notes.  During the first week of implementation, over 1450 patients had documentation performed in the on-line medical record by 28 nursing staff personnel and 52 providers including attending physicians, residents, physician assistants and nurse practitioners.
